Healthy Alternatives - Social Prescribing

Sometimes the problem you see the GP about is more complicated. Perhaps you need more than a prescription. To provide good healthcare we need to consider a mix of physical, mental and social issues. This is where the social prescribing service comes into play. Please see below for some information provided by our social prescriber, Will Shipp. If you feel the service is for you, speak to one of the Pioneer clinicians.

The scheme provides you with extra services and support within the community.  This involves exploring what is important to you, identifying local activities, groups etc that you can benefit from and then giving you the practical support you need to access them.  This may include creative activities, physical activities, counselling support, social activities, learning opportunities, training, employment or housing and money support!  There is also the opportunity to receive mentoring support should you and I feel that this would be beneficial to you.

Have a friendly conversation with us about any issue affecting your health and wellbeing. We can listen, support and help you to access a wide range of help or activities in your community - from gardening groups to benefits advice – and have lots of knowledge about what’s going on and where.  Our appointments are 45 minutes and we offer up to six sessions, so you’ll have plenty of time to explain your situation. To use this free service please ask your GP for a referral to Healthy Alternatives.

A message from the team:
Many things affect your health and wellbeing. GPs tell us that a lot of people visit them feeling isolated or lonely. Or they might be stressed out by work, money and housing problems. Sometimes it’s the stress of managing different long-term conditions.
That’s where social prescribing comes in. It starts with a conversation. It might be the conversation you’ve just had with your doctor. Or with another person in the practice team. They will refer you to a social prescribing link worker.
The link worker is there to listen to you, and put you in touch with the people and activities that might help you to feel better. Social prescribing can help you to have more control over your own health and find ways to improve how you feel in a way that suits you.
