Urgent Care Team


The urgent care team comprises a collection of healthcare professionals working together to help service the unpredictable, on the day demand. Their work comes from multiple sources; hospital discharges, out of hours handovers, 111 reports, urgent home visit requests, follow up from A+E attendances. Patients with acute illness unable to wait for routine, pre-booked appointments. 

The team comprises two duty GPs, nurse practitioners, paramedics, care coordinators, consulting GPs and admin support. In a typical working day the team will manage around 150 - 200 patient contacts. These represent the on the day demand which is over and above the many hundreds of pre-planned face to face appointments / calls / messages managed daily by the Pioneer team.

Care Coordinator


We have one care coordinator working at Pioneer. This role is varied but principally involves co-ordinating the handover and interface between primary (GPs) and secondary care (A+E and local hospitals). Sometimes people are discharged from hospital in something of a rush and the time spent by a dedicated team to coordinate the sharing of information between all the parties involved is time well spent.

Claire Mulligan
Claire joined the surgery in 2017.
Claire trained as a registered nurse at Bristol Royal Infirmary. 

After having her family she returned to work in the NHS in an administrative role for several years in primary care before joining Pioneer as Care Coordinator.

Claire has been instrumental in the implementation of our community volunteer driver scheme. Thanks to her kind work we are able to put patients in touch with a volunteer driver to bring them in to the surgery if they have no transport.

Paramedics


We are pleased to welcome a new profession to the staff mix at Pioneer. Esther, Jerry and Rob are paramedics with extensive experience of urgent care working in the ambulance service. Paramedics are experts at patient assessment, helping consult patients with acute illness.

Esther Hosznyak
Esther joined the surgery in April 2019.

​Esther has worked as a paramedic since 2014. Before joining PMG she worked for South Western Ambulance Service foundation Trust. Prior to that she worked for events medical companies, providing paramedic cover at music festivals. She also worked for Birmingham City University as a visiting lecturer in Paramedic Science where she gained a post graduate certificate in Learning and Teaching in Higher Education.

Jeremy Patton
Jeremy joined the surgery in April 2019.

Jerry has worked as a Paramedic for 17 years across the South Central region, most recently working as a Specialist Paramedic / Emergency Care Practitioner at two GP practices in Berkshire, providing clinic support at a drop in centre for the homeless and a frailty service for elderly patients.

Rob Audain
Rob joined the surgery in November 2019.

Rob worked as a Paramedic before joining the BrisDoc out of hours team as an urgent care practitioner.
