We encourage all our patients to take control of their lung health by attending their reviews every year. It is best to book an appointment with our Asthma Nurse Specialist to review your asthma. An alternative is to complete THIS FORM and return the form to reception at your surgery. A clinician will contact you if we need to discuss your asthma further. Thank you.
You may be asked to book for lung function testing prior to your COPD review. The nursing team will guide you on this. It is helpful if you can bring your inhalers and spacers along.
